Abstract: This paper presents a review of loss-of-containment incidents experienced by flexible pipe manufactured by Wellstream and the wider context of in-field failures for flexible pipe in general. A comparative assessment of steel pipeline and riser failure rates with flexible failure rates is also presented. Though the distribution of failures by mode as analyzed is specific to Wellstream, the failure modes themselves are typical for flexible pipe in general; furthermore, there is parity in the failure rates for Wellstreams product when measured against both the flexible pipe industry in general and, surprisingly, steel lines. Neither does there appear to be a difference between the failure rates for dynamic and static flexible lines.
